The amount of times I see functions and modules that are never called.

Or code that is commented out and left there forever.

I have a bit of a pet peve with this and "safety net" code as well. Just QA well, and if something breaks, fix it! Half the time the safety net is what breaks, or it hides a problem you need to deal with properly. Get that garbage out of my repo!

I would give emphasis on read.
Stumbling on it often, someone think it might be important and mislead him, stop someone from quick refactorings as he think it's more to be done... etc.
